  9. 19________. I, Larry Williams, being of sound mind, willfully and
  10. voluntarily make known my desires that my dying  shall not be
  11. artificially prolonged under the circumstances set  forth below,
  12. do hereby declare:

  14.  If at any time I should have an incurable injury, disease, or
  15. illness certified to be a terminal condition by two physicians who
  16. have personally examined me, one of whom shall be my attending
  17. physician, and the physicians have determined that my death will
  18. occur whether or not life- sustaining procedures are utilized and
  19. where the application of life-sustaining procedures would serve
  20. only to artificially prolong the dying process, I direct that such
  21. procedures be withheld or withdrawn, and that I be permitted to
  22. die naturally with only the administration of medication or the
  23. performance of any medical procedure deemed necessary too provide
  24. me with comfort care.

  26.  In the absence of my ability to give directions regarding the use
  27. of such life-sustaining procedures, it is my intention that this
  28. declaration shall be honored by my family and physicians as the
  29. final expression of my legal right to refuse medical or surgical
  30. treatment and accept the consequences from such refusal.

  33. emotionally and mentally competent to make this  declaration.
